Verification of Childhood Symptoms (VCS) Form
The Verification of Childhood Symptoms (VCS) form is part of Kantoko’s onboarding process and is required for PBS-subsidised ADHD medication.
It helps confirm that ADHD symptoms were present in childhood, as required by the Pharmaceutical Benefits Scheme (PBS).
1. Why It Matters
The PBS requires evidence that ADHD symptoms were present before the age of 13 to determine eligibility for subsidised stimulant medication.
Without this verification, stimulant medication can still be prescribed, but:
- You’ll pay the full private cost, and
- The PBS subsidy cannot be applied.
Completing the VCS form supports your eligibility for PBS-subsidised ADHD treatment and ensures prescribing complies with national guidelines.
2. How to Complete Your VCS
To complete your VCS form:
- Log in to your Kantoko account.
- Go to the Assessments section.
- Locate the VCS form under the “External” category.
- Enter the email address of someone who has known you since before the age of 13.
- Invite them to complete the form via the secure link sent to their email.
💡 Tip: If your nominated person doesn’t receive the invitation, ask them to check their spam or junk folder.
3. If You Can’t Have the VCS Completed
We understand that not everyone has someone available who knew them before the age of 13.
If that’s the case, you can provide alternative evidence to support your PBS eligibility.
Acceptable alternatives include:
- School reports noting attention or behavioural concerns
- Childhood psychological or educational assessments
- A statement from a sibling, extended family member, or close family friend who knew you as a child
- Medical records referencing ADHD-related symptoms or concerns
These documents help your clinician meet PBS evidence requirements where a VCS cannot be completed.
4. If You’re Having Issues
If you’re having trouble completing your VCS — for example:
- You don’t have anyone to complete the form or supporting documents
- The form was sent to the wrong email address
- You need to speak to a team member

5. Important Note
If no suitable substitute for the VCS form can be provided, Kantoko must comply with PBS requirements, and you may not be eligible for PBS-subsidised medication.
In these cases, medication can still be prescribed at private cost.
