Requesting Release of Complete Medical Records
If you need a release of further clinical records - including internal clinical notes related to your care - you'll need to request this in writing.
Our policy is to have your treating doctor review your notes before they're released to you.
To request your records:
-
Submit your request in writing to our support team, stating that you'd like your complete medical records. Email support@kantoko.com.au with:
- Your full name and date of birth
- The email address linked to your Kantoko account
- What you're requesting (e.g. full record, or a specific date range/document type)
- We may contact you to verify your identity before processing your request, to protect your privacy and prevent your records being released to the wrong person.
- We'll send you an invoice for the release of your complete medical records. This is $99, effective 6 August 2026. Requests submitted before this date are not charged.
- Once your invoice is paid, we'll aim to provide your records within a reasonable period. Since your records require a doctor's review before release, this can take up to 30 days.
We may not be able to release certain parts of your record if:
- It contains information about another person
- Disclosure could cause serious harm to your health or safety
