How to Request Your Records
- Submit your request in writing to our administration team (email or post).
- Please provide proof of identity (e.g., driver’s licence or Medicare card).
- You may request your full medical record or a summary.
- A reasonable fee may apply for preparation, copying, and secure delivery.
Why We May Seek Your Doctor’s Review
Although not required by law, our policy is to have your treating doctor review your notes before they are released. This ensures:
- The information is clinically appropriate and complete
- Any sensitive or complex details are accompanied by explanation
- The notes are accurate and up to date at the time of release
Timeframe for Access
We aim to respond to all requests within 30 days. If your request is urgent, please let us know and we will do our best to prioritise it.
Limits on Access
We may not be able to release certain parts of your record if:
- It contains information about another person
- Disclosure could cause serious harm to your health or safety
- It is subject to legal restrictions
If any information is withheld, we will provide written reasons and discuss alternative options with you.
