To help our clinicians prepare for your ADHD assessment, we’ll need a few documents uploaded securely through your Kantoko portal.
Step-by-Step Guide
Go to: https://app.kantoko.com.au/documents/
Click Add Document.
Complete the fields:

Document Name → Please name your file clearly using the format:
FullName_TestResult
Example: JohnDoe_ECG. - Choose File → upload your document (PDF, image, or Word file)
- Category → select the correct type from the dropdown (e.g. Referral, ECG, Bloods).
- Click Add document
- Repeat for each additional document.
💡 Tip: Upload each document separately under its correct category to help our clinicians review your information quickly.
Documents to Include
Please upload all of the following as documents:
- ECG: Include a clear image of the ECG graph.
- Referral: Your GP referral letter for ADHD assessment.
- Blood Tests. Please include the following pathology results:
- FBC (Full Blood Count)
- EUC (Electrolytes, Urea, Creatinine)
- LFT (Liver Function Tests)
- TFT (Thyroid Function Tests)
- B12 / Folate
- Iron studies
File Requirements
- Accepted formats: PDF, JPG, PNG, or DOCX
- Ensure files are legible and clearly labelled.
- Large scans can be compressed before upload (under 10 MB per file).
Tracking Your Vitals
You can add your vitals (blood pressure, heart rate and weight) directly in your Measurements in your Kantoko account. You can find this under "Medical Records".
Troubleshooting
If the upload button doesn’t respond:
- Try refreshing the page.
- Use Google Chrome or Safari.
- Check that your internet connection is stable.
